# Lower Back Pain: Common Causes and When to See a Physiotherapist

 Most lower back pain comes from strain on the muscles, joints and discs of the back, not from serious damage, and it usually improves within a few weeks. Staying active, simple exercises and good habits help most. See a physiotherapist if pain lasts more than two to three weeks, keeps returning or spreads down the leg.

Lower back pain is so common that most adults will experience it at least once. The World Health Organization describes low back pain as the leading cause of disability worldwide. In my practice in Ahmedabad, I see it in office workers, homemakers, drivers, students, new mothers and older adults alike.

The good news is that most back pain is not serious and gets better. Understanding why it happens and what helps can reduce worry and speed up recovery.

## How your back works

Your lower back, or lumbar spine, is made of five strong bones called vertebrae. Between them are soft cushions called discs. Small joints at the back of the spine guide movement, and layers of muscles and ligaments support everything. Nerves leave the spine and travel to your legs.

This is a strong and well designed structure. It is built to bend, twist, lift and carry. Pain does not usually mean something is broken.

## Common causes of lower back pain

### Non specific back pain

In most people, doctors cannot point to one single damaged structure. This is called non specific low back pain. It usually comes from the muscles, joints and ligaments of the back being overloaded or irritated. Common triggers include:

- Long periods of sitting, especially without breaks
- Lifting something heavy, or lifting while twisting
- A sudden awkward movement, such as bending to pick up a child
- Doing much more activity than usual, like a big cleaning day before Diwali
- Poor sleep, stress and low mood, which make pain feel worse

### Disc related pain and sciatica

Sometimes a disc bulges and irritates a nearby nerve. This can cause pain that travels down the leg, called sciatica. Disc bulges are very common, even in people without pain, and many settle with time.

### Age related changes

As we get older, the discs and joints of the spine naturally change. These changes are seen on X-rays and MRIs of most people over 40, including many who have no pain at all. They are a normal part of ageing, like grey hair, and do not always explain pain.

### Less common causes

A small number of people have back pain from other causes, such as a fracture after a fall, inflammatory conditions, infection or, rarely, cancer. This is why it is important to know the warning signs listed later in this article.

## Everyday habits that strain the back in Indian homes

- **Floor level work:** sitting on the floor to cut vegetables or wash clothes, and repeated bending to sweep and mop
- **Low kitchen counters:** standing bent forward for long periods while cooking
- **Two wheeler travel:** daily rides on bumpy roads, often with poor posture and a heavy bag
- **Lifting children:** often from the floor or while twisting
- **Long hours at a laptop:** often on a sofa or bed instead of a proper desk
- **Inactivity:** little regular exercise to keep the back and core strong

## What you can do at home

For new back pain without warning signs, these steps help most people:

1. **Stay active.** Gentle walking around the house or outside is better than bed rest. Rest briefly if needed, but avoid staying in bed for days.
2. **Keep doing normal activities** as much as you can, adjusting heavy tasks for a few days.
3. **Use heat** to relax tight muscles. A warm water bag or heat pack for 15 to 20 minutes can help.
4. **Try gentle movements,** such as lying on your back and rocking both knees slowly from side to side, or bringing one knee at a time towards your chest.
5. **Change position often.** Avoid sitting for more than 30 to 45 minutes at a time.
6. **Sleep comfortably.** Use pillows between or under your knees.
7. **Pain medicines:** if you need them, ask your doctor or pharmacist what is suitable for you.

## Myths about back pain

- **“I should rest until the pain goes.”** Long rest usually slows recovery.
- **“My MRI shows a disc bulge, so my back is damaged.”** Disc bulges are common in people without pain, and many shrink over time.
- **“I must never bend or lift again.”** Your back is designed to bend and lift. With good strength and gradual practice, most people return to normal activities.
- **“A belt will fix my back.”** Belts can give short term comfort, but long term reliance can weaken your own muscles.

## How a physiotherapist helps

At your first visit, I ask about your pain, your work and daily routine, and any worries you have. I check how your back moves, test the nerves to your legs, and look at your hips and posture. Then I explain what is going on in simple words.

Treatment usually includes:

- **Hands on therapy** to ease muscle spasm and stiffness
- **Exercises** to improve movement and build core, hip and back strength, which NICE guidelines recommend for low back pain
- **Advice for your daily life:** sitting, lifting, kitchen work, two wheeler riding and sleeping
- **A plan for flare ups,** so you know what to do if pain returns

## How long does back pain take to improve?

Many people feel much better within two to six weeks. Some have pain that lasts longer or keeps coming back. Long term back pain is also very treatable, though it usually needs a more structured exercise programme and attention to sleep, stress and activity levels.

## Preventing back pain from coming back

- Keep active with regular walking or other exercise you enjoy
- Do core and hip strengthening exercises a few times a week
- Take regular breaks from sitting
- Lift with your knees bent and the load close to your body
- Sleep well and manage stress, as both affect pain

## When to see a physiotherapist

Book an assessment if:

- Your pain has not improved after two to three weeks
- Pain keeps coming back every few months
- Pain spreads into your buttock or leg, or you have tingling in your leg
- You are afraid to move or bend because of pain
- Back pain is affecting your work, sleep or family life
- You want a clear exercise plan to prevent future episodes

## Warning signs that need a doctor

See a doctor urgently, or go to the emergency department, if you have back pain with any of these:

- Numbness or tingling around the genitals or buttocks
- New difficulty passing urine, or loss of control of the bladder or bowels
- Weakness in both legs, or weakness that is getting worse
- Back pain after a significant fall or accident
- Fever, feeling very unwell, or unexplained weight loss
- A history of cancer
- Constant pain that is severe at night and not eased by any position

## Frequently asked questions

Is it safe to exercise with lower back pain? 

For most back pain, gentle activity is safe and helps recovery. Start with walking and easy movements, and avoid heavy lifting for a few days. Increase activity as pain allows.

Should I use heat or ice for back pain? 

Heat often eases muscle tension and stiffness in the back. Some people prefer cold in the first day or two. Use whichever feels better, for 15 to 20 minutes with a cloth between the pack and your skin.

What is the best sleeping position for back pain? 

There is no single best position. Many people are comfortable on their side with a pillow between the knees, or on their back with a pillow under the knees.

Why does my back hurt more in the morning? 

Stiffness after a night of not moving is common and usually eases within 30 minutes of gentle movement. Morning stiffness lasting more than an hour, especially in younger people, should be checked by a doctor.
