## What is cardiopulmonary physiotherapy?

Cardiopulmonary physiotherapy, also called chest or cardiorespiratory physiotherapy, is treatment for people whose heart or lungs are not working as well as they should. “Cardio” means the heart and “pulmonary” means the lungs.

When breathing is difficult, people naturally move less. Moving less makes the muscles weaker, so the same task needs even more effort, and breathlessness gets worse. Cardiopulmonary physiotherapy breaks this cycle. It teaches you how to breathe more efficiently, how to clear mucus from your chest, and how to build stamina safely, so daily life becomes easier.

This treatment always works alongside the medicines and advice from your doctor or cardiologist. It does not replace them.

## Who it helps

- People with **COPD** or long term breathing problems who get breathless walking or climbing stairs
- People with **asthma** who want better breathing control, alongside their inhalers
- People recovering from **pneumonia, COVID 19 or a long hospital stay**
- People after **heart surgery or a heart attack**, once their cardiologist has cleared them for exercise
- People after **chest or tummy surgery**, to keep the lungs clear and prevent chest infections
- Older adults who feel tired and breathless with daily activities

## Common signs you may need it

- Breathlessness during simple tasks like dressing, bathing or walking to the next room
- A cough with mucus that is hard to clear
- Feeling tired and weak after an illness or hospital stay
- Avoiding activity because you fear getting breathless
- Your doctor has suggested pulmonary or cardiac rehabilitation

Call 108 or go to hospital at once if you have chest pain or pressure, sudden severe breathlessness, lips turning blue, or confusion.

## How I help

After a careful assessment, which includes checking your breathing pattern, oxygen level with a pulse oximeter, heart rate and how far you can walk comfortably, I may use:

- **Breathing control:** relaxed, slow breathing to reduce the feeling of breathlessness
- **Pursed lip breathing and positions of ease:** simple techniques to recover quickly after exertion
- **Airway clearance:** the active cycle of breathing technique and supported coughing to clear mucus
- **Graded exercise:** walking, leg and arm exercises that start gently and build up week by week
- **Energy saving tips:** pacing, planning and sitting for tasks like cooking and bathing
- **Education:** understanding your condition and knowing early warning signs

Throughout every session I monitor how you feel, your heart rate and oxygen level, and I stop or adjust when needed.

## Your first session

I ask about your diagnosis, medicines, inhalers, recent hospital visits and your daily routine. I check your breathing, chest movement, posture, oxygen level and walking ability. Then I teach you breathing control and one or two simple exercises. If you use inhalers, I may also check that your technique is helping you get the full benefit, and suggest you confirm it with your doctor.

## How many sessions you may need

Most people need six to twelve sessions over four to eight weeks, along with daily practice at home. Research on pulmonary rehabilitation shows that regular exercise over six to eight weeks can improve breathlessness, energy and quality of life. After the programme, staying active every day is what keeps the benefit going, and I help you build a routine you can maintain.

## Frequently asked questions

Is exercise safe if I get breathless easily?

Yes, when it is planned carefully. Some breathlessness during exercise is normal and safe. I start at a level that suits you, teach breathing control, and increase activity slowly while checking how you feel.

Do you replace pulmonary or cardiac rehab programmes?

No. If your doctor has referred you to a hospital rehab programme, please attend it. I provide physiotherapy at home or at my practice that follows your doctor's advice, especially if you cannot attend a programme.

Can breathing exercises help after COVID 19?

Many people feel breathless and tired for weeks after COVID 19 or pneumonia. Breathing exercises and graded activity can help you recover energy safely. Please tell me if you have chest pain, a racing heart or dizziness, so we can check with your doctor.
