## What is joint pain physiotherapy?

Joint pain physiotherapy is treatment that helps painful joints move better and hurt less. It focuses on the knees, hips, shoulders, ankles, wrists and small joints of the hands and feet.

The most common cause of joint pain in adults is osteoarthritis. In osteoarthritis, the smooth cartilage that covers the ends of the bones slowly becomes thinner, and the joint can become stiff, swollen and painful. Knee osteoarthritis is very common in India, especially in women over 45 and in people who spend long hours standing, squatting or climbing stairs.

Many people believe that a painful joint should be rested. In fact, the right kind of movement is one of the best treatments. International guidelines, including those from NICE in the United Kingdom, recommend exercise and education as the first and most important treatment for osteoarthritis, for every person, at every stage.

## Who it helps

I see many patients in Ahmedabad who:

- Have knee pain when climbing stairs, sitting cross legged or getting up from the floor
- Feel stiff in the morning and need a few minutes to “warm up” their joints
- Have been told they have arthritis and are worried about getting worse
- Have been advised a knee or hip replacement and want to try exercise first, or prepare well for surgery
- Have shoulder pain that stops them from lifting their arm, dressing or sleeping on that side
- Have stiffness in a joint after a fracture or after a plaster cast was removed

## Common signs you may need it

- Pain in a joint during or after activity, such as walking, standing or climbing stairs
- Stiffness after rest, for example in the morning or after sitting through a long function
- A grinding or cracking feeling in the knee
- Swelling around a joint
- Your leg feels weak or your knee feels like it may give way
- You walk less than before because of pain

## How I help

First I find out which joint structures are involved and what you find hardest in daily life. Then we work on these together.

- **Strength training.** Strong muscles around a joint act like a shock absorber. For the knee, I focus on the thigh and hip muscles. This is the most important part of treatment.
- **Range of motion work.** Gentle movements and stretches to reduce stiffness and help you bend and straighten fully.
- **Hands on therapy.** Joint mobilisation and soft tissue work to ease pain and make exercise easier.
- **Balance training.** To make you steadier and lower your risk of falls.
- **Activity planning.** How to pace walking, stairs, cooking, prayer and household work so you stay active without flare ups.
- **Practical advice.** Footwear, a walking stick if needed, chair and toilet height, and weight management where it helps.
- **Pain relief methods.** Heat, cold, TENS or taping when they help you move more comfortably.

## Your first session

At the first visit I ask about your pain, other health problems, medicines and any reports or X-rays you have. I watch how you walk, sit, stand and climb a step. I test the strength and movement of your joints and check the joints above and below the painful one, because a hip problem can cause knee pain and a neck problem can cause shoulder pain.

I then explain what is happening in your joint in simple words, start treatment, and give you a short home programme of exercises that are safe for you.

## How many sessions you may need

Joint pain usually improves gradually. Research on exercise for knee osteoarthritis shows that the benefits build over 6 to 12 weeks of regular exercise. A common plan is two or three sessions a week for the first few weeks, then weekly or fortnightly sessions while you continue your exercises at home.

The long term goal is for you to manage your joints yourself. When you finish, you will have an exercise routine you understand and can continue for life.

## Frequently asked questions

Can physiotherapy help knee pain if my doctor has suggested surgery? 

Many people with knee osteoarthritis improve with a good exercise programme, and guidelines recommend trying exercise first. Physiotherapy cannot change what is seen on an X-ray, but it can reduce pain and improve walking. The decision about surgery is always made with your orthopaedic doctor.

Is walking good or bad for knee arthritis? 

Walking is usually good for knee arthritis. The key is the right amount at the right pace. I help you find a level that keeps your knee active without causing a flare up, and build it up slowly.

Should I use hot or cold packs for joint pain? 

Heat often helps with stiffness and long standing arthritis pain. Cold can help with a fresh injury or a swollen joint. Use either for 10 to 15 minutes with a cloth between the pack and your skin. I will tell you which suits your joint.

Can you treat joint pain at home? 

Yes. Home visits are very useful for older adults or anyone who finds travel painful. I bring what I need and can also check your home for safety, such as stairs, the bathroom and the height of your chair.
