What is musculoskeletal physiotherapy?

Musculoskeletal physiotherapy is treatment for problems in your muscles, joints, bones, ligaments, tendons and the nerves around them. Doctors often call this group of problems “MSK” conditions. They are the most common reason people see a physiotherapist.

The World Health Organization says musculoskeletal conditions are the leading cause of disability around the world, and lower back pain is the single biggest cause. The good news is that most of these problems get better with the right movement, the right advice and time. Physiotherapy guides that process so you recover faster and with more confidence.

My aim is simple. I want to find out why you have pain, reduce it, and help you return to the things you need and enjoy, such as work, housework, walking, prayer, sport or playing with your children.

Who it helps

Musculoskeletal physiotherapy helps people of every age. In my practice in Chandranagar, I often see:

  • Office workers with neck, shoulder or back pain from long hours at a laptop or desk
  • Homemakers with back or knee pain from lifting, bending and standing for long periods
  • Older adults with stiff joints and pain that limits walking or climbing stairs
  • Young people and adults with sports injuries from cricket, running, gym training or badminton
  • People with a frozen shoulder who cannot lift their arm to comb their hair or reach a shelf
  • Two wheeler riders with neck and lower back strain from daily travel on Ahmedabad roads

Common signs you may need it

You may benefit from musculoskeletal physiotherapy if you notice:

  • Pain in your back, neck, shoulder, elbow, hip or knee that has lasted more than a few days
  • Stiffness when you wake up, or after sitting for a long time
  • Pain that travels down your arm or leg, or tingling in your hands or feet
  • Weakness, or a feeling that a joint may “give way”
  • Difficulty sleeping because of pain
  • Pain that keeps coming back every few months

If you have sudden severe pain after a fall or accident, numbness around your private parts, loss of bladder or bowel control, fever with back pain, or unexplained weight loss, please see a doctor straight away. These are warning signs that need medical care first.

How I help

Every treatment plan starts with listening. I ask how the pain started, what makes it better or worse, and what you want to get back to. Then I check how you move, test your strength and flexibility, and look for the real cause of the problem.

Depending on what I find, treatment may include:

  • Hands on therapy to ease tight muscles and stiff joints, such as soft tissue release and joint mobilisation
  • Exercise therapy to build strength, flexibility and control, starting gently and increasing step by step
  • Posture and ergonomic advice for your desk, your kitchen, your bed and your two wheeler
  • Electrotherapy such as TENS or ultrasound, where it helps with pain in the short term
  • Taping to support a joint during recovery
  • Education so you understand your pain and know what to do if it comes back

I do not believe in long treatment plans with no clear goal. I set targets with you, track your progress at each visit, and change the plan when needed.

Your first session

Your first session lasts about 45 to 60 minutes. Please bring any reports, X-rays or prescriptions you already have. Wear loose, comfortable clothes so I can see and move the painful area easily.

In the first session I will:

  1. Ask about your problem, your health history and your daily routine
  2. Check your posture, movement, strength and the painful area
  3. Explain what I think is causing your pain, in simple words
  4. Start treatment on the same day
  5. Teach you two or three easy exercises to do at home

You will leave with a clear plan and an idea of how many sessions you may need.

How many sessions you may need

The number of sessions depends on the problem, how long you have had it, and how regularly you do your home exercises. As a general guide:

  • A recent muscle strain or mild back pain may improve in 3 to 6 sessions
  • Long standing neck or back pain often needs 6 to 10 sessions over a few weeks
  • A frozen shoulder or a complex injury may need regular sessions over two to three months

Everyone recovers differently. I review your progress regularly and will tell you honestly when you are ready to continue on your own.

Frequently asked questions

Do I need an X-ray or MRI before physiotherapy?

Most back, neck and shoulder pain does not need a scan before starting physiotherapy. I will check you carefully at the first visit. If I see any sign that needs a scan or a doctor's opinion, I will tell you clearly and refer you.

Will the treatment be painful?

Treatment should not be very painful. Some exercises may feel uncomfortable at first, and hands on therapy can feel firm. I always explain what I am doing and adjust the pressure to what you can manage.

Can I do exercises at home instead of coming every day?

Yes. A home exercise plan is a big part of treatment. Most people come two or three times a week at first, then less often as they get better and can manage the exercises on their own.

Is online physiotherapy useful for back or neck pain?

Online sessions work well for follow up visits, exercise plans and posture advice. For a new injury or when hands on treatment is needed, I suggest a visit at my practice or a home visit first.
