What happens at a home visit?

The first visit

The first visit usually takes about an hour. I:

  1. Read reports, discharge summaries and doctor’s instructions
  2. Ask about the condition, daily routine and goals
  3. Check movement, strength, balance and walking
  4. Look at the home setup for safety
  5. Start treatment
  6. Teach a short exercise routine to the patient and family

Follow up visits

Follow up sessions usually last 30 to 45 minutes. I review progress, adjust exercises, add new challenges and answer questions. I share honest updates with the family and, if you wish, with your doctor.

How to prepare for a home visit

  • Keep reports, scans and the discharge summary ready
  • Clear a small space where the patient can lie down, sit and walk a few steps
  • Have a firm chair with arms available
  • Ensure the patient wears comfortable, loose clothing
  • If possible, have a family member present, especially at the first visit

This depends on the condition. After surgery or a stroke, two to five visits a week may be needed at first. For older adults working on strength and balance, two or three visits a week for a few weeks, followed by a home routine and review visits, often works well. I will suggest a plan at the first visit and review it regularly.

Many patients use a mix. For example, home visits in the first weeks after surgery, then practice visits as they become more mobile.

A typical home visit week after knee replacement

To give you a clear idea, here is what a common home visit plan can look like in the first weeks after a knee replacement:

  • Monday: check swelling and wound area, bending and straightening exercises, walking practice with a walker
  • Wednesday: progress strengthening exercises, practise getting in and out of bed and the toilet safely, first steps on stairs with support
  • Friday: longer walking practice, balance exercises, review the home routine with the family

Between visits, the patient does short exercise sessions two or three times a day, with help from the family as needed. As the patient improves, visits reduce and exercises become more challenging.

Tips for family members

  • Keep a simple diary of exercises done each day, and any pain or swelling
  • Encourage the patient to do as much as they safely can
  • Help with walking practice only as I have shown you
  • Ask questions at every visit, nothing is too small to ask
  • Look after yourself too. Caring for a family member is tiring, and your health matters.

Safety during home visits

Safety is always the first priority. Before any new activity, such as standing or walking, I check blood pressure if needed, make sure the space is clear, and position myself and family members to support the patient. Exercises are progressed only when the patient is ready, and I explain to the family which activities are safe to practise alone and which need supervision.

When to see a physiotherapist

  • A painful, swollen calf
  • Fever, a red or leaking surgical wound, or sudden confusion
